It was most likely me. 

Towards the end,  I was pressed to take an quick incoming call and when I
returned it was all over.

It seems that someone put their phone into hold mode. I tried to find out
who it was so I could mute it, but didn't make it in time. Apologies to
members for the way the call ended.
