Very helpful edits Michael.  Thanks.  They take care of my concerns
regarding the dba role.  BTW, there are several places where the global
replacement of dba to operator were inappropriate: feedback was changed
to feeOperatorck.
 
Also, I would suggest that all table rows be formatted to disallow rows
to break across pages unless a row is simply too wide.
