Please find attached an updated version of the outstanding issues document 
which now also includes the main issues identified by Paul/James following 
their review of the draft Final Report that were not yet covered in the list.

P.S. Paul/James, please note that the line numbers identified did not seem to 
match the relevant section, so please let me know if there is anything 
missing/incorrect. I have focused on the issue identified (either by topic or 
recommendation number) instead of the line number.
