Questions #5 and #6 would be less theoretical if a useful sample of 
domains used were available. At the moment, pre-coffee, I suggest the 
following tuple:
   name tld iana

e.g.

   fromthefloor us 439

That's a domain I picked up for my wife to use for bloggers at the DNC 
convention next month.
