Dear Amber, dear Klaus:

I recall NPOC leadership at some point had agreed (was it in Dakar, Michael or 
Debbie?) to update this form so that it would be a joint Cy/SG form, in this 
case a joint NPOC/NCSG Application Form. Has this been done? and if not may I 
ask you and Klaus to produce a draft and run it for endorsement by our EC 
before uploading on the NPOC website? Thanks.

For convenience I join our current form and the updated NCUC/NCSG joint form.
