So, would it be fair to say that if no human followups happened (and
they would not be as incentivized as end points who are direct
affected to notice/capture these errors), then the measured rate of
false positives is understating the "true" level?

Perhaps we need to discuss this more, as there are potentially going
to be millions of domain registrants who would want to be whitelisted
against any takedown system, especially assuming that the takedown
list operators are not going to fully compensate innocent sites that
are falsely accused.
