Hi,

I hope I got it all.

I will put out another version on Thursday i get new content and substantive 
corrections.

In addition to section 3.6 mentioned earlier, I am also waiting for possible 
new language  for section 3.6 relating shared costs/risk pools.

And of course the call for contributions to the FAQ sections.
