Aside from the obvious advantage that owners of generic words such as book, computer, medicine (and that's just the english variants) have, there's nothing preventing 2nd (and 3rd, and 4th...) level registrations from happening now. This does, of course, radically change the face of the existing registar landscape. C|net is already doing this with store.com.
