Let's face it, the number of "adult" oriented web sites on internet is predominant. While nobody can be forced to use .xxx suffix for their name, the majority of the owners of "adult" web sites will willingly choose .xxx for the same reason they put disclaimers "18+ only" and rate with RSACi. They want to be separated and if .xxx is used to prevent children from accessing their web sites, they will volunteer.
