                I believe name.space's approach is by far the most sensible, new gTLDs should be added in direct response to user demand. Perhaps the way to control the creation could be achieved by using something similar to the CFV system for Usenet.

Ultimately any hierarchical system is doomed to overcrowding in the popular 'top level' nodes, so this problem will recur again and again in the future.

Since DNS is an arbitrary mapping layer atop IP addressing, take full advantage of the abstraction layer and extend it massively.
