In short, I like the name Mikey suggested but respect Jonathan’s preference for 
“metrics”. So here are some proposed edits/options for your consideration:

“Data Collection and Reporting to Inform Policy Making”
or
“Data Collection and Reporting for Policy Making”
or
“Collection and Reporting of Data and Metrics for Policy Making”
